Effective September 2, 2011 the Portsmouth Location, on Frederick Boulevard next to the Wal-Mart will be closed. Patient records will be moved to our Virginia Beach location at the Sentara Princess Anne Health Campus. Patients of our Portsmouth location can continue to see Dr. Kanji and Dr. Pham at the Virginia Beach location if they wish. Please make sure to check if your insurance is accepted at the Virginia Beach location as coverage does differ. You may do so by checking the Insurance listing under the Locations link. If patients wish to receive a copy of their records, please click here and fill out the "Medical Record Release" form and follow the instructions.

WE WILL MAIL A HARD COPY OF YOUR RECORDS FREE OF CHARGE UNTIL SEPTEMBER 30, 2011. AFTER SUCH DATE, A $10.00 FEE APPLIES. FAXED OR EMAILED RECORDS WILL REMAIN AT NO CHARGE.
